Session 3: Policy in Practice: Advance Care Planning and Decision-Making

October 17 | 1pm ET
Sponsored by

This session will discuss all levels of advanced planning and decision-making that patients and their families can tackle.  While the hope is for early decision-making and advance care planning, far too often critical decisions are made far too late.  There are also hurdles to ensuring proper paperwork and permissions are gained and accepted.

This session includes two presenters who focus on end-of-life decision-making.  Learn about Physician Orders for Life Sustaining Treatment (POLST) and the work that they are doing to educate patients and move those critical forms out of folders and into the cloud, so they can be where the patient needs them. Additionally, we will learn about the growth of right-to-die legislation and the growth in the United States. For both of these speakers, we will consider the impact to home-based advanced illness, palliative and hospice providers and their patients.

Presenters
  • Wendy Smith, Member, Compassion and Choices Board of Directors
  • Susan Nelson, MD, FACP, FAAHPM, Member, National Physician Orders for Life-Sustaining Treatment (POLST) Paradigm Leadership Council
