ElevatingHOME Honors Leaders in Home-Based Care at 2018 National Leadership Conference

Washington, DC, March 7, 2018 ElevatingHOME today honored recipients of the 2018 ElevatingHOME Outstanding Leadership Awards at the National Leadership Conference in Washington, DC. The annual ElevatingHOME Awards program recognizes the contributions of innovative organizations and leaders in the home-based care industry.

“It is vital that we recognize members who go above and beyond to spearhead innovation our industry, and who elevate quality and access to home-based care,” said Tracey Moorhead, President and CEO of ElevatingHOME.

The 2018 ElevatingHOME Awards and recipients are as follows:
Program of the Year Award: Recognizes an ElevatingHOME provider member organization that implemented an innovative program, project and/or practice to improve the agency.
Recipient: Visiting Nurse & Hospice Care (VNHC) in Santa Barbara for PHorum: Perspectives in Healthcare
About: Launched in 2014, PHorum: Perspectives in Healthcare is a free community education program and annual symposium featuring healthcare topics critical to Santa Barbara County. Each year VNHC's PHorum brings together healthcare experts and stakeholders, community leaders, and the public at large in meaningful, progressive dialogue about relevant and timely healthcare issues.

Leader of the Year Award: Recognizes an individual employed by an ElevatingHOME provider member organization for their innovative contributions as a program leader.
Recipient: J. Mark Baiada, Founder and Chairman, BAYADA Home Health Care
About: Under Mark's leadership, BAYADA has become a role model for personal care delivery, quality, compliance, employee recruitment and retention, and innovation. In addition to BAYADA's well-documented success, Mark also shares his and BAYADA's expertise across the country. He has served on the boards of multiple national home care providers organizations including the Partnership for Quality Home Health Care, the Alliance for Home Care Quality and Innovation, and most recently ElevatingHOME, where he provides substantial wisdom and insight.

Partner of the Year Award: Recognizes an organization who has made significant efforts to advance home-based care through partnered work with a home-based care provider.
Recipient: American Academy of Home Care Medicine (AAHCM)
About: AAHCM is the leading advocacy and technical support behind the Independence at Home (IAH) CMMI demonstration, working with agencies across the country to grow home-based care. They have worked with ElevatingHOME on IAH and other mutual policy and research priorities to expand the advocacy and data to underscore the efficiencies of home-based care. AAHCM’s advocacy and partnership are increasing the numbers of patients cared for in the home while expanding research around the effectiveness of home-based medicine.
